Health Insurance Effective the 1st of the month after start date
Dental Insurance Effective the 1st of the month after start date
EyeMed Effective the 1st of the month after start date
Life Insurance Effective the 1st of the month after your 30 day benefit enrollment period ends
40% Long Term Disability Effective date is your start date
60% Long Term Disability Effective date is the date you enroll in this benefit
Accident Insurance Effective the 1st of the month after start date
Hospital Insurance Effective the 1st of the month after start date
Critical Illness Insurance Effective the 1st of the month after start date
Leave of Absence If non-FMLA leave is 30 days or less, continue billing for all benefits and bill medical at full premium
Leave of Absence If non-FMLA leave is over 30 days, bill medical full premium and cancel all other benefits
