On Tuesday, June 15, Wellesley Public Schools will hold a second dose COVID-19 vaccine clinic for students. The clinic will take place from 2:00 p.m. to 5:30 p.m. in the Wellesley High School cafeteria.
This clinic is provided by Pelmed Pharmacy and is a follow up to one held in May where students received their first vaccine dose.
Appointments can be reserved by utilizing this link Vaccine Clinic #2 . After registering you will receive a link with a username and password. When you click on my vaccine in the email and log in, you will see a screen with all of the information you will need to complete. Click next to print the consent form. Please be sure to print and sign the consent form and have your student bring it with them to the clinic. Parent or Guardian signatures are required for any students under the age of 18.
All students will be vaccinated free of charge regardless of insurance coverage. If insured, please enter insurance information in the online registration. Once registered online, there is no need to bring an insurance card to the clinic.
Additional Information:
- Students must bring their vaccine card that they received at the previous clinic with them
- You may register your child for this vaccine clinic even if they received the first dose elsewhere - there must be 21 days separating vaccines
- You may register your child for their first vaccine at this clinic but you will need to locate another site for the second vaccine
- Students must come with their signed consent form in hand - it cannot be on a phone
